No pharmacist–patient relationship through the website

Visiting this website, reading its content, or sending an inquiry through the form does not create a pharmacist–patient or provider–patient relationship. That relationship begins only after you schedule and complete an initial visit and we agree to provide care.

We cannot provide clinical advice, diagnoses, or prescriptions in response to website inquiries. Please do not include sensitive medical details in the form.

William Wong, PharmD, is licensed as an Advanced Pharmacist Practitioner in the State of California only (APh #11633; California Pharmacist #52556). Clinical services, including telehealth visits, are available only to patients who are physically located in California at the time of service.

Care is provided within the scope of California Advanced Pharmacist Practitioner authority. Conditions outside that scope are referred to your primary care physician or an appropriate specialist. Details are in the Telehealth & Jurisdictional Disclosures on our homepage.

Clinical services are governed separately

These Terms apply to your use of the website. If you become a patient, your care is also governed by the intake forms, consents, and policies you receive before or at your first visit, including telehealth consent and our Notice of Privacy Practices. Any fees and payment terms will be explained to you before services are provided. If those documents conflict with these Terms regarding clinical care, those documents control.

Acceptable use

You agree not to:

• Use the website for any unlawful purpose
• Submit false information or impersonate another person
• Send spam, malicious code, or automated submissions through the inquiry form
• Attempt to gain unauthorized access to the website or its systems, or interfere with its operation

The KLW Consulting name and logo, and the website's text, design, graphics, and photographs, are owned by or licensed to KLW Consulting and are protected by copyright and trademark law.
