Privacy Notice

How we handle your information.

This notice explains what information this website collects, how we use it, who we share it with, and the choices you have.

Effective date: September 27, 2026  ·  Last updated: September 27, 2026

Who we are and what this covers

This Privacy Notice explains how KLW Consulting, LLC ("KLW Consulting," "we," "us") handles personal information collected through this website, klwconsulting.health, including the general inquiry form.

It does not replace the privacy protections that apply once you become a patient. Health information you share during clinical care is covered separately, as described in Your health information below.

Information we collect

Information you give us

When you use the inquiry form, we collect:

  • Your name
  • Your email address
  • Your phone number, if you choose to provide it
  • The text of your question or message

You may also give us information when you call us or contact us by other means.

Please don't send sensitive medical details through the form. The form is for general questions only. For clinical questions, call us at 925-888-0738. For a medical emergency, call 911.

Information collected automatically

Like most websites, our web hosting provider automatically records basic technical information when you visit, such as your IP address, browser type, device type, the pages you request, and the date and time of your visit. This information is used to operate, secure, and troubleshoot the website.

This website does not use advertising cookies, analytics trackers, social media pixels, or similar tools that follow your activity across other websites.

How we use your information

We use the information described above to:

  • Respond to your inquiry by email or phone
  • Schedule an appointment if you ask us to
  • Operate, maintain, and secure the website, and prevent spam and abuse
  • Comply with legal, licensing, and professional obligations

We do not use information from the inquiry form for advertising or marketing to third parties.

Who we share it with

We share personal information only with the following categories of third parties, and only as needed to run this website and respond to you:

  • Website hosting and email service providers that host this website, process form submissions, and deliver inquiry messages to us
  • Google Fonts, which delivers the typefaces used on this website. When your browser loads a page, it connects to Google's servers, which receive your IP address and browser information. See Google's Privacy Policy.
  • Government authorities or other parties when required by law, court order, or professional licensing requirements, or when needed to protect someone's safety
  • A successor organization if our practice is merged, sold, or transferred, subject to the protections in this notice

We do not sell your personal information, and we do not share it with third parties for their own marketing purposes.

Your health information

If you become a patient, health information you provide during your care is medical information protected under California's Confidentiality of Medical Information Act (California Civil Code § 56 and following) and, where applicable, the federal HIPAA Privacy Rule.

How that information is used and disclosed, and your rights to access and amend it, are described in our Notice of Privacy Practices. You also receive a copy at your first visit, and you can request a paper copy at any time by calling 925-888-0738.

As required by law, your primary care physician is notified whenever your medication therapy is initiated, adjusted, or discontinued through this practice.

Your choices and rights

You may ask us to:

  • Tell you what personal information we hold about you from this website
  • Correct information that is inaccurate
  • Delete inquiry information we no longer need, unless we must keep it for legal or professional reasons

To make a request, contact us using the details in Contact us. We may need to verify your identity before acting on a request. Requests about medical records are handled under our Notice of Privacy Practices.

California "Shine the Light"

California Civil Code § 1798.83 allows California residents to ask whether a business has disclosed their personal information to third parties for those parties' direct marketing purposes. We do not make such disclosures.

Do Not Track signals

Some browsers send a "Do Not Track" signal. Because this website does not track visitors over time or across other websites, and does not allow third parties to do so for advertising, there is no cross-site tracking for such a signal to switch off. We treat all visitors the same way whether or not the signal is sent.

Google Fonts, described above, receives basic technical information when fonts load, but we do not use it to track you.

Security and retention

This website uses an encrypted (HTTPS) connection. We use reasonable administrative and technical safeguards to protect the information we collect. No method of transmission over the internet or email is completely secure, which is why we ask you not to include sensitive medical details in the inquiry form.

We keep inquiry information only as long as reasonably necessary to respond to you, to maintain appropriate business records, and to meet legal and professional obligations. Patient records are retained as required by California law.

Children's privacy

This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through it. A parent or legal guardian should contact us on behalf of a minor. If you believe a child has sent us information through the website, contact us and we will delete it.

Changes to this notice

We may update this Privacy Notice from time to time. When we do, we will post the revised version on this page and update the effective date at the top. If we make material changes, we will also place a notice on our homepage for a reasonable period before the changes take effect.
